Home visits are taken on a case by case basis and can be scheduled by emailing VitaCoreHolistic@gmail.com.

Additional Transportation Charge (distance is calculated from Lombard, IL) :

-$125 for 1-30 min distance from Dr. Haque's home.

-$250 for 31-60 min distance from Dr. Haque's home.

-Anything beyond that will be charged at $2 per minute of driving time to the location.
